To provide episodic andchronic care to patients residing in SNF/LTC sites. The nurse practitioner (NP) assistsindividuals and families with their care needs by obtaining health and medicalhistories, performing physical examinations, and assessing healthproblems. The NP cares for patients withacute and chronic life limiting illnesses by managing therapeutic regimens,prescribing medications and performing other studies and activities that aredeemed appropriate. New and follow-uppatients will be seen and evaluated by the NP and physician input will beobtained as necessary as described in the nurse practitioner practice guidelines. The NP will assume SNF/LTC care managementresponsibilities and will facilitate communication with the patient, thepatient's family or significant other and the referring physician. The NP will work in collaboration with thepatients' attending physicians at the SNF/LTC facility, as well as providecontinued collaboration with the patients' primary care provider(s).
Patients evaluated bythe NP include those patients referred to a SNF/LTC site that is attended bythe Extended Care Team. The patientpopulation includes patients 50 years of age and older.
- Maintain current certification inCPR.
- Masters ofScience. Certification as Adult, Familyor Geriatric Nurse Practitioner
- APRN licensure,Nurse Practitioner and MA RN licensure. Maintain current controlled substance licenses (Federal and State)
Two years experience with adult or geriatric Care NPpreferred
