32-Hour Inpatient Palliative Care Social Worker (LICSW) - Salem Hospital

Overview

Summary:

As a member of the Palliative Care Service at North Shore Medical Center, the Palliative Care Social Worker will provide comprehensive clinical and psychosocial assessment and intervention for patients with serious illness. The social worker will work in collaboration with other members of the palliative care team to conduct and participate in family conferences, particularly when there are complex psychosocial needs. The social worker must be comfortable working in a fast-paced, inpatient setting, where the structure of the day may change depending on patient, family and staff needs. The social worker will provide consultation and education to staff regarding social work practice, palliative care and end of life issues. In this role he/she will provide clear communication, both written and verbal, to the multidisciplinary staff (including physicians, nurses, case managers, social workers and chaplains) regarding patient care concerns and end of life issues. In keeping with the NASW Standards for Social Work Practice in Palliative and End of Life Care (http://www.socialworkers.org) the social worker will play a multidimensional role as clinician, educator, advocate, and team consultant. He/she will collaborate with appropriate clinical staff regarding patient and family centered discharge planning, identify high-risk psychosocial factors of patient/families that impact health status, decision making, and outcomes and help staff understand the influence of those factors upon the course of medical care.

Essential Duties and Responsibilities:

  • Performs comprehensive psychosocial assessments of hospitalized patients with serious illness. Assists the health care team to integrate psychosocial factors into treatment plans: Ability to analyze significant psychological, physical, emotional, social and economic issues of patients/families and to effectively communicate the relevance of such to other health care providers. Ability to identify, analyze and suggest interventions for difficult patient/family management situations.
  • Provides social work intervention including counseling and support to assist patients/families. Demonstrates ability to form empathic relationships with patients/ families who have significant emotional adjustment and coping issues. Enhance community partnerships to better link patient and family needs with appropriate community resources; coordinating and collaborating with the iCMP Program and NSPG Extended Care Teams.
  • Perform follow up visits and provide continuity of care for our palliative care patients who are readmitted; and, where indicated, offer follow up support for smoothing out transitions of care from the acute care setting to subacute care.
  • Effectively addresses quality of life and end of life issues within a culturally competent framework to support the equitable delivery of excellence in health care to all patients and families.
  • Assess patient/ family’s level of understanding, barriers to learning, and to provide age-specific and culturally competent education as needed. Able to effectively assess and discuss the psychosocial issues of palliative care to patients/families
  • Arranges, attends and actively participates in patient/family conferences, in person, as well as via available telehealth resources.
  • Maintains patient records and other documentation in a manner consistent with standards of clinical care.
  • Assists in data collection for the Palliative Care Service and participates in quality improvement initiatives.
  • Attends Palliative Care Team rounds and all required departmental meetings.
  • Collaborates with Case Management staff.
  • Uses community resources and hospices available to support palliative care for our patients in the outpatient setting.
  • Participates in educational programs for NSMC staff as an expert on psychosocial issues in palliative care.
  • Works with members of the team to provide bereavement follow up for our palliative care patients.
  • Coordinates and facilitates regular Palliative Care Service support meetings to help ensure the health of team members.
  • Participates in Palliative Care Service follow up phone calls of patients after they are discharged from NSMC.
  • Helps to ensure that we have appropriately documented Serious Illness Conversations, as well as appropriately scanned- in copies of other advance care planning documentation (such as completed health care proxies, MOLST forms, Living Wills, Guardianship paperwork) into the Electronic medical record for all patients seen by our service for palliative care consultation.


Qualifications

Skills:

  • Strong clinical skills to assess patient- centered and family coping while demonstrating a strong ability to connect with diverse population of patients and families.
  • Excellent interpersonal skills.
  • Ability to set priorities
  • Excellent communication (both oral and written) with ability to communicate effectively with all members of the care team.
  • Strong organizational skills and ability to set priorities
  • Basic understanding of medical conditions, treatments and interventions that are commonly encountered during palliative care consultations.
  • Strong computer skills

Experience:

  • Required: A minimum of 3 years of previous experience in an inpatient hospital or hospice setting.
  • Required: Knowledge of palliative care, end of life issues and hospice
  • Preferred: ICU or oncology experience
  • Preferred: Prior experience in Palliative Care or hospice care strongly preferred

Education:

  • Masters in Social Work from accredited program required

Licensure:

  • Licensed Independent Clinical Social Worker (LICSW) required
  • Current Massachusetts license required
  • Must be actively working towards or presently certified in Palliative and End of Life Care by the National Association of Social Workers. Attainment of certification within 2 years
