Overview

To provide episodic and chronic care to patients residing in SNF/LTC sites. The nurse practitioner (NP) assists individuals and families with their care needs by obtaining health and medical histories, performing physical examinations, and
assessing health problems. The NP cares for patients with acute and chronic life limiting illnesses by managing therapeutic regimens, prescribing medications and performing other studies and activities that are deemed appropriate.
New and follow-up patients will be seen and evaluated by the NP and physician input will be obtained as necessary as described in the nurse practitioner practice guidelines. The NP will assume SNF/LTC care management responsibilities and will facilitate communication with the patient, the patient's family or significant other and the referring physician. The NP will work in collaboration with the patients' attending physicians at the SNF/LTC facility, as well as provide continued collaboration with the patients' primary care provider(s).

Patients evaluated by the NP include those patients referred to a SNF/LTC site that is attended by the Extended Care Team. The patient population includes patients 50 years of age and older.


Qualifications

REQUIRED:
- Demonstrated competency in providing comprehensive health care including evaluation, diagnosis and treatment
- Knowledge of accreditation standards and compliance requirements
- Use of Electronic Medical Record
- Knowledge of minor surgical procedures and equipment
- CPR Certification for Healthcare Professionals
- Up to 1 year experience as a nurse practitioner preferably in a family medicine, primary care or pediatric setting.
- Graduate of an accredited school of nursing with a Master's degree
- Current Massachusetts state license as a registered nurse practitioner
