The Care Manager, as an integral member of primary care team, strives to enhance the delivery of patient care services along the continuum of care. Utilizing a patient-centered approach, the Care Manager collaborates with primary care and specialty physicians and practice staff, acute care and community-based providers, patients and their caregivers to develop, implement, monitor and evaluate a high-quality, cost-effective care plan for designated patients in the ambulatory setting. The Care Manager meets patients’ needs efficiently and expeditiously by continually working to improve the patient experience, engaging patients in self-care and efficiently managing the utilization of health care resources.
