Our team is seeking a 40-hour RN Care Manager to work as part of our iCMP (Integrated Care Management) Program and support two of our primary care practices in a hybrid capacity (4 days in office, 1 day remote) after the successful completion of our onboarding and training program. As an Internal Medicine office, we provide physical examinations, routine health screenings and preventative care for adults.
Utilizing a patient-centered approach, the Care Manager collaborates with the entire care team (primary care and specialty physicians, practice staff, acute care and community-based providers, patients, and their caregivers) to develop, implement, monitor and evaluate a high-quality, cost-effective care plan for designated patients in the ambulatory setting. The Care Manager meets patients’ needs efficiently and expeditiously by continually working to improve the patient experience, engaging patients in self-care and efficiently managing the utilization of health care resources.
