General Summary

Our team is seeking a 40-hour RN Care Manager to work as part of our iCMP (Integrated Care Management) Program and support one of our primary care practices in a hybrid capacity (4 days in office, 1 day remote) after the successful completion of our onboarding and training program. The practice is located at 331 Highland Avenue in Salem, MA. As an Internal Medicine office, we provide physical examinations, routine health screenings and preventative care for adults. The office hours are Monday through Friday from 8:00am to 5:00pm.

Utilizing a patient-centered approach, the Care Manager collaborates with the entire care team (primary care and specialty physicians, practice staff, acute care and community-based providers, patients, and their caregivers) to develop, implement, monitor and evaluate a high-quality, cost-effective care plan for designated patients in the ambulatory setting. The Care Manager meets patients’ needs efficiently and expeditiously by continually working to improve the patient experience, engaging patients in self-care and efficiently managing the utilization of health care resources.

Qualifications

Required

  • Completion of an accredited nursing degree program    
  • Minimum 5 years nursing experience
  • Registered Nurse with a current, valid license to practice in the state of Massachusetts.

Preferred

  • Bilingual (English/Spanish)
  • Minimum of 2 years of acute care experience. Other experience may include post-acute care, homecare, care coordination and/or case management experience.
  • Bachelor’s Degree 
  • Certification in case management or other applicable professional certification, such as CCM or ACM.
