General Overview

We are looking for a full-time Navigator with clinical licensure (ideally RN, PT, or OT) to work Monday through Friday from 8:00am to 5:00pm. This role will be based out of our Medical Office Building on the Salem Hospital campus with travel to other locations on the North Shore.
The Clinical Care Transition Navigator coordinates care for a select group of patients to guide them through the issues related to the transition from one level of care to another. The Navigator is responsible for identifying the appropriate post-acute care setting ensuring that sufficient services are in place in that setting for a safe transition. This coordination of care is accomplished through a collaborative process working with the patient, family members and the acute interdisciplinary team. The Clinical Care Transition Navigator will engage both patients and their families while providing objective information and support throughout the care continuum. The interdisciplinary team usually consists of, but is not limited to the physicians/NPs, inpatient case managers, social workers, nursing, home care, and rehab staff. The goal for this position is to optimize patient care transitions thereby reducing unnecessary post-acute utilization and decreasing the likelihood of readmission.

Qualifications

Required
  • Minimum 5 years of experience, including at least 2 years post-acute, care coordination and/or case management experience.
  • Bachelor’s Degree and/or graduate of an accredited program related to licensure is required
  • Licensed in the State of Massachusetts as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), Social Worker (LICSW), Physical Therapist Assistant (PTA), or Licensed Practical Nurse (LPN)
Preferred
  • Skilled Nursing Facility experience.
  • Master’s Degree
