Resource Nurse - Internal Medicine, Salem

Overview

We are looking for a full-time RN or LPN to join our Internal Medicine conveniently located at 331 Highland Ave, Salem, MA! This Resource Nurse will assist with One Call Resolve patient triage in addition to assisting the Care Team. The office is open Monday through Friday from 8:00am to 5:00pm.

What you will do:

Under the direction of the PCP and Practice Manager, the Resource Nurse is responsible for ensuring that patients receive timely, high quality and efficient health care and support services within and outside of the Patient Centered Medical Home. The Resource Nurse:

  • Works closely and collegially with physicians, associate providers, clinical and administrative staff
  • Collaborates to enhance the delivery of patient care services along the continuum of care
  • Works closely with the nurse care manager and assists in the management of patients
  • Promotes patient satisfaction by interacting with patients in a professional, friendly manner consistent with their needs

Job Specific Responsibilities: Under the direction of the Primary Care Provider and in collaboration with the PCMH Care Manager:

Direct Patient Care:

  • Assists with phone triage for complex urgent calls.
  • Assists Care Team when office emergencies arise:
  • Monitors patient and provides support to patient/family
  • Provides hand off to EMS and ED (in collaboration with Provider)
  • Assists the Care Team to remain in flow:
  • When necessary and if MA unavailable:
  • Administers injections, medications and provides complex wound care
  • Assists with minor surgery , office laboratory and other tests.
  • Instructs patients regarding preparation for various tests and procedures.
  • Communicates and interprets providers clinical information and instructions to patients.
  • Provides patient education/coaching and gives handoff to Care Manager if appropriate.

Care Coordination:

  • Maintains a list of community resources
  • Obtain authorization for services/equipment
  • Initiates Home Care Referrals and assists with coordination of home care services
  • Arrange follow up after hospitalization
  • Assists with Post ED/Hospital/Post Acute Follow up calls
  • Initiates referral for education using internal and external sources
  • Perform other duties as related to medical home as assigned
  • Communicates appropriately and effectively with patients
    • Recognizes and respects cultural diversity
    • Adapts communication to the patient’s understanding
    • Employs professional telephone and interpersonal techniques
    • Serves as a communication liaison between the physician and the patient
  • Functions as an integral part of the patient care team
    • Interacts in a professional, cooperative way with other practice staff
    • Accepts and delegates responsibilities in a positive manner
    • Engages in constructive problem solving with others in the practice as problems arise
    • Attends and participates in practice staff meetings
  • Promotes patient/customer satisfaction by interacting with patients in a professional, friendly manner consistent with their needs
  • Adheres to Human Resource policies with respect to attendance and dress code
  • Adheres to North Shore Physicians Group customer service philosophy in all patient interactions

Qualifications

What you will need:

Required:

  • Excellent critical thinking skills
  • Excellent communication skills
  • Document all patient communications and clinical treatments accurately and appropriately
  • Comply with established risk management and safety procedures
  • Comply with all local, state and federal privacy and confidentiality rules and regulations
  • Sound judgment and problem solving ability
  • Ability to prioritize tasks in a complex and busy environment and see them through to completion
  • Accuracy and attention to detail
  • Maintain or obtain CPR certification and participate in site-based CPR drills.
  • Educates the patient within the scope of practice and as directed by supervising care manager in health maintenance, disease prevention, disease management and compliance with the patient’s treatment plan.
  • Helps the provider and patient identify community resources for health maintenance and disease prevention.
  • Assists provider and other office staff in population management using available tools
  • Use of Electronic Medical Record
  • Completion of an accredited Nursing degree program

One of the following:

  • 3 years of experience as an LPN OR
  • 1-2 years experience as RN

One of the following:

  • RN with an active license to practice in Massachusetts OR
  • LPN with an active license to practice in Massachusetts

Preferred: Bilingual speaking (Spanish/English)
