Resource Nurse/RN - NSPG Cardiology (Salem) - EXPERIENCE REQUIRED


This is an exciting position for a registered nurse to join a thriving cardiology practice and to help to start an Advanced Congestive Heart Failure Program. The RN for Congestive Heart Failure (CHF) is an active member of the CHF Team and works closely with Department staff to deliver efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication and coordination of resources. Nursing Care is provided to individuals and groups of patients under the guidance and direction of the CHF Medical Director.

In this role, the RN uses the nursing process and evidence-based practice to help develop patient-driven care plans. This includes collaboration with services internal and external to the NSPG to facilitate care transition in order to effectively meet the patients' needs. The RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. This RN also assesses for alterations in patient's health status, implements interventions based on disease management approved orders, with primary objectives of ER/Hospital avoidance, improved quality of life, improved clinical outcomes and care coordination. The RN must demonstrate leadership in delivering and improving holistic care to the adult and geriatric population through collaborative strategies.

The registered nurse is the designation given to an individual who is a graduate of an approved registered nursing program, and who is currently licensed as a registered nurse in Massachusetts. The Registered Nurse functions within the framework specified by the nursing statutes and regulations of the Commonwealth of Massachusetts.


  • Participate in the treatment plan for patients with heart failure, by attending the bi weekly interdisciplinary team meetings
  • Coordination of patient care across the continuum of care including inpatient, outpatient, LTC, hospice, palliative care, and home care.
  • Work with hospital quality department to insure the CV department is meeting regulatory and quality guidelines and goals
  • Provide education to staff and affiliated providers about program
  • Availability to see patients in clinic setting when needed. Acute assessment and triage skills necessary to insure best practice care. This may include giving direct advice, directing the patient to a hospital emergency facility, scheduling an appointment with a time frame consistent with the assessment and clinical presentation.
  • Maintain records of vital statistics and other pertinent data of patient. Receives patient lab results and other data, enters and organizes this data into the patient medical records according to North Shore Medical Center and Salem Family Health Center protocols.
  • Effectively respond to and communicates urgent or time sensitive information to the appropriate providers in a timely manner.
  • Measure and/or records patient histories, height, weight, temperatures, blood pressure and pulse as appropriate for the visit.
  • Communicate and interpret providers clinical information and instructions to patients; with specific direction from the provider including but not limited to instructing patients regarding medication, use of home testing or other equipments or medical supplies, communicating testing results, counsels patients on health maintenance related issues (e.g. cholesterol, control, blood pressure control, etc).
  • Act as a liaison and advocate for patients with external agencies and institutions to ensure that all necessary care is received.
  • Provide care management and ensures continuity of care for individuals and families through the development and implementation of tracking systems and follow up procedures.
  • Instruct patients regarding preparation for various tests and procedures.
  • Oversee prescription refills and new medication as directed by the providers.
  • Inform patients of test results and/or providers recommendations in person or via phone or mail.
  • Document this contact in the patients’ record.
  • Record review must be done at the end of each day.
  • Assist with the oversight and coordination of quality controls.
  • Trains new or less experienced medical assistants and nurses.
  • Works with other staff members in a cooperative and supportive fashion.
  • Attends meetings and continuing educations opportunities as directed.
  • Seek to expand skills and professional knowledge.
  • Perform other tasks as requested by the clinic preceptor or program manager.


Required: Graduate of an accredited school of nursing
Required: Registered Nurse in the Commonwealth of Massachusetts
